Bias Incident Reporting
The Bias Incident Reporting Form provides the entire Miami University community a mechanism for reporting incidents involving acts of hate or bias due to age, ancestry, color, disability, sex or gender identity, pregnancy, national origin, race, religious belief, sexual orientation, military or veteran status. This form should not be used if you are currently in a situation where immediate medical, police, psychological, or other emergency services are needed. In case of an emergency, call 911 to get the appropriate services. Only use this form in non-emergency situations. Even if you are not in an emergency situation but believe that you or someone else needs medical attention or has been the victim of a crime, you should contact the appropriate medical or police services. Please understand that even if you request that Miami University take no action beyond recording this incident, Miami University may, depending upon the circumstances, respond institutionally to situations that may threaten the safety of anyone mentioned or potentially affected by this report.
Statement of Confidentiality

Only Miami University constituents may use the system. The system requires users to logon with a Miami University User ID (unique ID). Should a report indicate or imply that the user or someone else is in danger, the report will be turned over to appropriate Miami University personnel. All information will be held confidential to the extent allowed by law.
