Reminder to return eligibility certificate for benefits plan

Employees enrolled in the high deductible health plan with the health savings account (HSA) for 2009 are reminded to complete and return the certificate of eligibility by Dec. 19. The certificate was e-mailed last week.

It is important to complete the certificate and submit it to benefit services by 5 p.m. Dec. 19 or a delay of funding for the health savings account will occur.

Employer and employee funds will not be released to the health savings account until the signed certificate is received. If an employee is not eligible and cannot complete the certification, he or she will be moved to the traditional plan.
